Over 16's only, under 16's need to be accompanied by a legal guardian who will need to be present in the treatment, and also the guardian will be required to sign the consultation form.
Consultation form required to be completed for each treatment prior to treatment start
Pregnancy 12 weeks plus for treatments
Need to be made aware of any serious or terminal illnesses as some treatments may be contraindicated.
Patch tests are required 48 hours in advance of any tinting
Need to be made aware of any surgery in the last 12 months as treatments may need to be tailored
